Position Summary:

Reporting to the Manager of Patient Flow & Transitions the Social Worker assists patients to address the impact of illness and treatment by providing services which meet the medically related psychosocial needs of the patients and provide support for their families. As an integral part of the interprofessional health care team they conduct assessments and appropriate interventions to aid the patient in achieving optimum recover/rehabilitation and quality of life. Social workers support the patient’s and families discharge planning goals, working in collaboration with the interprofessional team and our Community partners, to enable transition to life after non-acute hospital care. In collaboration with the interprofessional hospital team and Community partners, the Social Worker facilitates the patient’s movement to the next appropriate level of care


 



Qualifications:

 

§ Minimum five years healthcare related Social Work experience -preferred.

§ Perform a comprehensive psychosocial assessment of patients

§ Educate patients/SDM’s on the roles of health care team members; assist patients and families in communicating with one another, to members of health care team and with community service agencies;

§ Participate in interprofessional team treatment planning, including interprofessional Team Rounds;

§ Involve patient and family in plan of care, advocating for the patient and family, respecting wishes and keeping them well informed. Advocate for the dignity and respect of all individuals. Reassess plan of care on an ongoing basis.

§ Bring forth appropriate cases to Complex Discharge Rounds, presenting patient case, barriers to discharge, and work with CCAC, community partners and hospital staff to develop actionable/accountable plan of care.

§ Initiate and organize case conferences and family meetings, including community partners and interprofessional team members as required;

§ Guided by their expressed wishes, assist patients and families in the development of a viable discharge plan;

§ Educate hospital staff on patient psychosocial issues;

§ Provide or facilitate referrals to community partners on behalf of patients and families;

§ Assist patients and families to navigate resources such as health insurance, disability income, housing applications, etc.;

§ Provide counselling to assist individuals and families to cope and manage grief, depression and anxiety related to their physical, emotional, financial and psychological well-being;

§ Strong therapeutic and assessment skills.

§ Ability to work independently as well as with other professionals and community agencies.

§ Proven computer skills in Microsoft Office/ Excel.

§ Ability to assume leadership role, as required.

§ Fluent in French language – preferred

Education and/or Certification:

§ Masters of Social Work degree – required

§ Current Certification of Registration issued through the Ontario College of Social Workers & Social Service Workers
