Position Summary:
Reporting to the Program Manager, the Registered Nurse is responsible and accountable for the provision of safe, effective, client-focused nursing care to clients and families. In collaboration with the client, family and other team members, the Registered Nurse is responsible for ongoing assessment and monitoring of the client’s health status, identification of client/family needs and the development of plans to meet those needs. The Registered Nurse is responsible for the application of current theory and research in the delivery of care consistent with the vision and values of the organization.
Qualifications:
Deliver a full range of high-quality nursing services to clients
Organize and collaborate in the planning and implementation of nursing best practices with Registered Nurses and the interprofessional team
Screen all incoming referrals to ensure eligibility criteria are met
Perform comprehensive intakes and in-home assessments as required, including health assessments, medication review, recommendations of care, health teaching, safety promotion, falls prevention and care giver education
Help clients with addressing difficulties through skills assessment and instructions, goal-oriented planning, and educating
Perform cognitive testing
Provide education to clients and caregivers on dementia prevention strategies, disease progression and approaches to supporting ongoing care and management
Monitor, record, and report symptoms and changes in clients’ conditions
Record client’s medical information and vital signs
Consult and coordinate with interprofessional team members to assess, plan, evaluate and recommend care plans
Interpret, and evaluate diagnostic tests to identify and assess clients’ condition
Protects clients and employees by adhering to infection-control policies and protocols, medication administration and storage procedures, and controlled substances regulations
Education and/or Certification:
Bachelor's of Science Nursing Degree – required
Current Certificate of Registration issued through the College of Nurses of Ontario – required
B.C.L.S certification – required
MOCA Certification – required
Valid Drivers License in good standing and access to an insured vehicle– required
3-5 years of experience working with the geriatric population – required
P.I.E.C.E.S certification – preferred
Certificate in Gerontology – preferred
Ability to work in an interprofessional program care delivery model
Thorough knowledge of nursing principles, procedures, and practice in accordance with the Standards of Nursing Practice of the College of Nurses and regulations of the Regulated Health Professions Act
Excellent problem-solving skills and the ability to think innovatively and independently.
Excellent interpersonal skills and effective communication skills
Proficiency in French language is an asset
